       NY Notice and Acknowledgement of Pay Rate and Pay Date Forms - The Wage Theft Prevention Act (took effect on April 9, 2011.  The law requires employers to give written notice of pay rates (1) to each new hire and (2) to all employees by February 1 of each year.  Additionally the employer must keep a signed copy of this document for a minimum of six years.  Below are the necessary forms based on payment status:
